Digital clubbing, look at his fingertips and notice the disparity of color between his red swollen arm and the right pale thin arm. A photo with both arms down would be nice but I don't it interesting anyhow.

Wouldn’t surprise me if it’s related to malnutrition due to the various famines North Korea has endured. He looks like he probably would have been in his teens or 20’s during the Arduous March famine in the 90’s, so unless he was from a powerful/influential family then he likely suffered from malnutrition while a conscript in the army.

It kind of looks like his dopp bag strap is just cutting off circulation at the wrist, and maybe his watch is too tight.

Most of white people I know including myself have light "bottom" side of arm, palm facing down, and darker top side. For obvious reason, sun

Both sides of his other are still much lighter..
