Well there are a lot of these metro trains going around Sydney all the time, and very seldom does anything like this happen. I just don’t want people thinking that metro trains aren’t safe.

Well, I was thinking more about the other ones.
When I was a kid, the doors on our trains in Perth were open on trains all the time. They could close, but it was all manual and nobody bothered unless it was raining. So, I'm not too bothered that anyone was in actual danger.
Transport Minister John Graham said the incident was the result of a manual override of the door and the matter would now be the subject of an investigation.
It sounds from this like the door was out of service and not talking to the train, but some dimwit manually opened it somehow.
I remember it happening often enough on Melbourne trains in the 90s/00s. Not often, but not a surprise that it happened. I suppose the headline "Passengers mildly concerned at train traveling with door open. Passengers apply common sense and keep away from the open door while the train is in motion" Isn't quite the same headline.
