Not the onion. Gender affirming care has frequently initially been intended for cis people and trans people just happen to be a side benefit.

(Big) Part of the problem is how few cis people understand that.

I heard a Christian apologist once unironically claim his male breast reduction wasn't gender-affirming care. Admittedly, he's an apologist so he might have just been lying.
